LIBRARY USAGE GUIDELINES
Students
Small groups (2-5 students) may be sent to the library with a pass. The pass should have the full names of students and the teacher name legibly written. There should be times noted for when the students left the classroom and when they are expected to return. Teachers should make advance arrangements or call ahead to ensure library space is available. Larger groups should be accompanied by the teacher.
Classes
Substitute teachers should not send students to the library unless prior arrangements were made between the teacher and the librarian. Teachers should not schedule a library day when they will be absent. For unexpected absences, the library day will be rescheduled upon the teacher's return.
**Changes to the library schedule and the above guidelines may occur when/if administration deems necessary.
- Students must have an official hall pass from their scheduled teacher to visit the library during school hours. Students who wish to visit the library during lunch must complete a lunch pass request.
- All visitors should sign-in at the library circulation desk.
- Students must check out materials with the librarian before taking them from the library. Reference materials may only be used while in the library
- Materials are checked out for a two-week period, but may be renewed for more time.
- Students are allowed two checkouts at a time.
- Students will be charged the cost of replacing any lost or damaged materials.
- Overdue or non-returned materials will result in the loss of check-out privileges.

Classes
- Classes must be scheduled with the librarian at least three days prior to the date requested when library instruction is needed.
- Visit the library circulation desk to view the calendar and schedule classes. The calendar can also be viewed at www.fdhslibrary.weebly.com
- Complete a Library Information Center Request form at the time of scheduling. This will help the librarian to assist teachers and to provide necessary library resources for lessons.
- Please do not expect to schedule two or more consecutive weeks. This will not allow for equal access of the library.
- One class during each period can be scheduled in the library.
- Teachers with scheduled classes must accompany students to the library and remain with students for the duration of the library visit.
- Students must have an Internet Permission form (Student Access Release and Authorization form) on file to use the internet. This is a SCS Policy.
- Classes will not be scheduled if the librarian is out for professional development. This will ensure library services are being rendered to students while maintaining the integrity of the library program.
